New Client Forms

New clients - Please feel free to fill out your medical history form or teeth whitening consent form before your appointment. 

One version is fillable online and can be emailed as an attachment to sparklingsmilesfredericton@gmail.com, or you are welcome to print a copy and fill in by hand and bring to your appointment with you.

If you are attending an appointment for teeth whitening only, we do require both a medical history and teeth whitening consent to be filled out.
